There are several reasons women consider breast augmentation. The procedure is used to enhance the body contour of a woman who, for personal reasons, feels her breast size is too small, it can also be utilized to correct a reduction in breast volume after pregnancy, balance a difference in breast size or as a reconstructive technique following breast surgery.
"By inserting an implant behind each breast, surgeons are able to increase a woman's bust by one or more bra cup sizes," explains P. Craig Hobar, M.D. "After surgery, breasts appear fuller and more natural in tone and contour."
Dr. Hobar uses all forms of implants, but believes that in a high percentage of patients, anatomic or "teardrop-shaped" - implants provide the most natural look. "The teardrop implants most closely resemble the ideal breast shape and can produce beautiful results." he says. "However, every patient is unique, and some patients are best fitted with the round implants because of the original shape of their breasts."

To aid in achieving the most natural possible results, Dr. Hobar uses the endoscopic technique with most of his breast augmentation patients. This procedure is completed through an inconspicuous incision in the armpit. "The transaxillary, or armpit approach, is an outstanding way to perform breast augmentation," he says. "The incision is virtually undetectable after it heals and there are no scars on the breast." The instrument used, an endoscope, allows very precise dissection - essential for obtaining the correct placement and shape.
"I am opposed to the 'TUBA' or transumbilical approach, because there is very little ability to precisely dissect the pocket for the implant." Dr. Hobar says. "The endoscope-assisted transaxillary approach has all the benefit - no scar on the breast - but is much more accurate and artistic in my opinion."
The anatomical breast implant consists of a silicone outer shell filled with saline solution. To better meet each individual woman's needs, breast implants come in different sizes. In his initial consultation, Dr. Hobar discusses the patient's individual preferences that he can take them into account in choosing the right implant size and shape.
Dr. Hobar performs the procedure in an operating room at the Dallas Day Surgery Center. The surgery usually lasts one to two hours. Dr. Hobar makes the incision and creates a pocket for the implant controlling the shape the implant and breast will take. Then the implant is placed in the pocket, filled and positioned. Finally, the incision is closed with stitches that dissolve after about a week to 10 days. The surgery generally heals within several weeks.
Dr. P. Craig Hobar is a plastic surgeon board-certified by the American Board of Plastic Surgery practicing in Dallas, Texas.
